How to Configure Questionnaires
Questionnaires are forms for the Portal and AssureGO+.
They can be shorter than the main module forms and they allow you to collect key information from people who are not Users in the main system.
Navigate to Questionnaires by clicking Settings -> Portal -> Questionnaire.

You will see a list of default Questionnaires that come with the system.
The List View shows the Name of the Questionnaire, the Module it is based on, how many sections it has, if Attachments can be uploaded if Auto Process is enabled, and the User Input Language.
To create a new Questionnaire, click New Record.

Fill in the required details. The fields with a red (*) next to them are mandatory.
Note that you can optionally attach an iQ Template to the questionnaire, which will be made available to be completed on the Portal or in AssureGO+.

You can also configure the Questionnaire to allow users to upload a maximum of 100 attachments.

You can configure actions to be enabled within the Portal Questionnaire.

You can also change the Questionnaire Icon from the Image Upload section. (This will be visible on the Portal Dashboard)
The Image must be either .png or .jpg. Maximum size of 500px x 500px.
When you are happy with the settings, click Save & Close.

Clicking on the menu cog for the Questionnaire will bring up a menu for creating the body of the form.
(Some of these options are dependent on the module that you based the form on so some may not appear)
Click Sections from the list.

Below is an example of sections within a Vehicle Incident portal form.

When you are first creating a Questionnaire, this view will be blank.
To create a new section, Click New Record.

Enter a section heading and click Save & Close.

Click the menu cog on your Section and select the Fields option.
You can create fields within your section by clicking the New Record button.

Select the property/field from the drop-down and fill in the details.
The caption can be customised, but it will still link to the original field (named in the drop-down) in the main system.

You can use the breadcrumbs at the top of the page to go back to your Questionnaire.

Adding a Questionnaire to a Portal
Navigate to Settings -> Portal -> Portal Dashboard.

Click the menu cog next to your portal and select Edit.

Scroll down to the Questionnaires section and expand.

Click the Add button, select your Questionnaire and press Select.

Click Save & Close at the bottom of the screen.

Your Questionnaire will now be live on the portal.
