This article explains how to configure a Job Hazard Analysis (JHA) Questionnaire (also referred to as a Job Safety Analysis (JSA) or Safe System of Work (SSOW)) for use in AssureGO+ (the Evotix mobile-accessible progressive web application for field-based data capture). To be able to view the JHA form on the AssureGO+ app, you will need to configure the Questionnaire and then add the Questionnaire to the Dashboard. This article covers the full configuration process, including creating the Questionnaire, adding the JHA-specific Submodules (Job Hazard Analysis Hazards and Controls, and PPE Image), configuring fields, and publishing the Questionnaire to a Portal Dashboard.
Cross-module scope: This article is scoped to the Portal and AssureGO+ configuration area. The JHA Questionnaire is anchored to the Job Hazard Analysis sub-module within the Risk module in the Assure main system. For information on creating a JHA record in the Assure main system, refer to the companion article: Creating a Hazard Analysis (JHA, JSA, SSOW).
Prerequisites
- An active Assure user account with the Manage Portal Supervisor Privilege.
- The Questionnaire is anchored to the Job Hazard Analysis module. Fields added to a Questionnaire map directly back to the corresponding field in that module, meaning data submitted through the Questionnaire is stored in the module record.
- If Auto Process is enabled and all mandatory module fields are present in the Questionnaire, submitted records are created directly in the module without manual intervention. If mandatory fields are missing or Auto Process is disabled, records enter the Portal Queue for manual review.
Steps
Step 1 - Navigate to Questionnaire Configuration
Go to Settings > Portal > Questionnaire
Step 2 - Create a New Questionnaire Record
Select the New Record icon.
Complete the following fields on the new Questionnaire record. Fields marked with a red asterisk (*) are mandatory:
- Name (required): Enter a name for the Questionnaire.
- Module (required): Select Job Hazard Analysis.
- User Input Language: Select the appropriate language.
- Auto Process: Enable if all mandatory module fields are included in the Questionnaire, to allow records to be processed directly into Assure without passing through the Portal Queue.
Attachments
You can configure the Questionnaire to allow users to upload a maximum of 100 attachments. To enable attachments, tick the attachments checkbox and set the maximum number of files permitted. When enabled, users completing the form in AssureGO+ can upload photos, videos, or files directly against the record, for example, to photograph a hazard or upload supporting documentation.
Actions
You can configure Actions to be enabled within the Portal Questionnaire. When Actions are enabled, users completing the JHA form in AssureGO+ can raise an Action (a follow-up task assigned to a user within Assure to complete a corrective or preventive measure) directly from within the form, without needing to access the main Assure system. Actions raised in AssureGO+ are managed through the Action Manager in Assure once the record has been submitted.
Icon
You can change the Questionnaire Icon from the Image Upload section (this will be visible on the Portal Dashboard). The image must be either .png or .jpg, with a maximum size of 500px x 500px.When you are happy with the settings, click Save and Close.
You can also configure the Questionnaire to allow users to upload a maximum of 100 attachments. Step 3 - Add Sections and Fields
1. Clicking on the menu cog for the Questionnaire will bring up a menu for creating the body of the form. Some of these options are dependent on the module that you based the form on, so some may not appear. Click Sections from the list.
2. When you are first creating a Questionnaire, this view will be blank. To create a new section, click New Record. Enter a section heading and click Save and Close.
3. Click the menu cog on your Section and select the Fields option.
4. You can create fields within your section by clicking the New Record button. Select the property/field from the drop-down and fill in the details. The caption can be customised, but the field will still link to the original field in the main system.
Step 4 - Add the Hazards and Controls or PPE Image Submodules
The JHA module has two unique Submodules that can be added to the AssureGO+ Questionnaire: the Job Hazard Analysis Hazards and Controls submodule and the Job Hazard Analysis PPE Image submodule. These are specific to the JHA module and cannot be added to other questionnaire types.
Adding Submodules
Once you have created the Questionnaire, you can add in the Hazards and Controls or the PPE Images. To do this, select the Cog and then Submodules.
Select New Record. On the next page you can see the 3 Submodules you can select:
- Signature
- Job Hazard Analysis Hazards and Controls
- Job Hazard Analysis PPE Image When you are selecting some of these Submodules, you may also see the option to add Actions and Attachments to them.
Configuring the Job Hazard Analysis Hazards and Controls Submodule
When you have added the Job Hazard Analysis and Controls Submodule, select the Cog and then Fields. Select New Record. You will then be able to add the following fields. Fields marked with a (*) are mandatory and will need to be added if you have set the Questionnaire up to Auto Process:
- Step (mandatory)
- Potential Hazards (mandatory)
- Control Measures
- Additional Control Measures
- People Affected
- How is the Person Affected
- Pre-Control Risk Factor
- Post Control Risk Factor You can then rename the field, add guidance, and make the field mandatory
Configuring the PPE Submodule
With the PPE Submodule you will not need to add any additional fields as they are already populated.
Step 5 - Add the Questionnaire to the Portal Dashboard
1. Navigate to Settings > Portal > Portal Dashboard.
2. Click the menu cog next to your portal and select Edit.
3. Scroll down to the Questionnaires section and expand. Click the Add button, select your Questionnaire and press Select.
4. Click Save and Close at the bottom of the screen.
Once you have added all the fields and Submodules to the form you can then add the form to the Portal Dashboard. Users will now be able to access and create records via AssureGO+.
Step 6 - How the JHA Form Appears in AssureGO+
Once the Questionnaire has been configured and added to the Portal Dashboard, users can access and complete it in AssureGO+.
Completing Hazards and Controls
Select the Add JHA Hazard and Controls button. The fields will show in a pop-up.
The pop-up form contains the following fields:

Completing PPE
For the PPE fields, select Add PPE. When you select the PPE from the list, you will also be able to add any comments you have added to the Picklist Data Dictionary for that field.
Note: PPE picklist values are managed in the Assure main system under Settings > System Configuration > Picklist Data Dictionary > Job Hazard Analysis PPE Categories.
