Job Hazard Analysis (JHA) - Assure Go+
To be able to view the JHA Form on the Assure Go+ App, you will need to configure the questionnaire and then add the Questionnaire to the Dashboard.
How to Configure the JHA Questionnaire.
Go to Setting, Portal, Questionnaire.

Select the New Record icon.





Adding the Hazard and Controls or the PPE Images.
Once you have created the Questionnaire you can add in the Hazard and Controls or the PPE Images.
To do this select the Cog and then Submodules.

Select New Record.
On the next page you can see the 3 submodules you can select.

When you are selecting some of these Submodule you may also see the option to add Actions and Attachments to them.
When you have added the Job Hazard Analysis and Controls Submodule, select the Cog and then Fields.

Select New Record.
You will then be able to add in the following fields (please note – anything with a * next to it is a mandatory field and will need to be added if you have set the Questionnaire up to Auto Process).

You can then rename the field, add guidance, and make the field mandatory.
With the PPE submodule you will not need to add any additional fields as they are already populated.
On Assure Go+.
Select the Add JHA Hazard and Controls.

The fields will show in a pop up.

For the PPE fields select Add PPE.

The follow screen will show.

When you select the PPE from the list you will also be able to add any comments you have added to the Picklist Data Dictionary for this field.
Once you have added all the fields and submodules to the form you can then add the form to the Portal Dashboard.
Users will now be able to access and create records via Assure Go+.
