General Risk Detailed Report (Excl. Scoring)

This article explains the General Risk Detailed Report (Excl. Scoring) (a standard system report available in the General Risk Assessment module of Assure that outputs all record detail including iQ questions, answers, actions, signatures, and guidance, but explicitly excludes iQ scoring data). The article covers how to enable or disable the report via Module Setup, how to run the report from an individual General Risk Assessment record, and a complete reference guide to every section and element that appears in the report output. The report draws data from the General Risk Assessment record itself, from the associated Project record (for logo and address fields), from the iQ Template (for questions, guidance, and answers), and from System Settings (for colour coding). The report supports image display, caption localisation, and HTML rendering in Template Guidance.

Cross-module scope: This report spans the General Risk Assessment module, Project record data, iQ Template configuration, and System Settings. It is configured via Settings > System Configuration > Module Setup.

Prerequisites

  • The user performing the configuration must have access to Settings > System Configuration > Module Setup and the Manage Module Settings Supervisor Privilege.
  • To run the report on a record, the user must have the Report permission enabled in their Role Permissions for the General Risk Assessment module.

Enabling and Configuring the Report in Module Setup

The Detailed Report (Excl. Scoring) has been designed for the General Risk Assessment module. The report can be enabled and disabled within the Module Setup of the General Risk Assessment module. 

1. Navigate to Settings > System Configuration > Module Setup.

2. Scroll to the Risk section and click the Cog menu for the General Risk Assessment module, then select Reports.

3. From the Reports screen, you can customise the report title, add or remove reports from the Standard Reports list, and enable images to be shown on reports from the cog menu by default.

Running the Report

When you select the Reports option from the cog menu of a General Risk Assessment record, you will be able to run the Detailed Report (Excl. Scoring) for that record.

Report Content Reference

The following sections describe every element included in the Detailed Report (Excl. Scoring) output.

Cover Page

Left Logo
  • The image is sourced from an attachment on the record's associated Project record with description "Report Image".
  • The image will only display if an image is available.
Right Logo
  • If there is a report image specified for the record's Org Unit, that image will be used.
  • Otherwise, the Report image specified in System Settings will be used.
Page Title
  • Displays the iQ Template Name.
Cover Page Image
  • Recommended image ratio: 16:9.
  • The image is sourced from an attachment on the record's associated Project record with description "Site Image".
  • If there is no image, the Page Title is vertically centred within the available space.
Cover Page Fields
  • Cover Page fields will only be displayed if they have a value saved in the record.
  • Cover Page fields will reflect any caption configuration (caption change/display in interface).
  • Cover Page fields will localise depending on the language setting of the user running the report.
  • Cover Page fields (apart from Address) are sourced from the General Risk Assessment record.
Address/Org Unit
  • Sourced from the "Address" field on the record's associated Project.
  • If Address is empty, the Org Unit from the General Risk Assessment record will be shown.

 

Risk Matrix

The Risk Matrix page will only be displayed if there is a selected risk factor for the record.
The page includes:
  • Page Title: taken from the Matrix Factor Caption Label
  • Matrix Guidance Notes of the Risk Matrix used in the record
The record's risk rating is displayed as:
  • Matrix Factor Caption: Selected cell value/score (coloured)
  • Severity Axis Caption: Severity Row Caption/score
  • Likelihood Axis Caption: Likelihood column Caption/score

 

Additional Fields

The Additional Fields page will only be displayed if the iQ template has additional fields and at least one has a value recorded.
  • No Title
  • Each field will only display if there is a value recorded against it

Signatures

The Signatures page will only show if there are signatures recorded against the record.
Title
  • The Signatures title is non-configurable.
  • The Signatures title is localised.
Signature
  • Signature captions (Full Name, Description, Signature) can be configured via Caption Maintenance.
  • Signature captions (Full Name, Description, Signature) are localised.

Question Actions

Title
  • "Question Actions" title is non-configurable.
  • Titles will localise depending on the current user's language setting.
Ordering
  • Actions created against iQ Questions are displayed first.
  • Actions will be sorted and grouped into iQ sections (in template order).
  • Within the section, Actions are ordered by Priority (high to low).
Section Header
  • The iQ section name will be a header.
  • Section header (and actions) will only display if Actions have been created within that section.
Action field captions displayed:
  • Action
  • Action Detail
  • Priority
  • Current Status
  • For User
  • Category
  • Due Date
  • Colour coding of Priority, as configured in System Settings

  • Colour coding of Status, as configured in System Settings

     

Record Actions

  • "Record Actions" title is non-configurable.
  • "Record Actions" title will localise depending on the language setting of the user running the report.
  • Record-level Actions are sorted by Priority (high to low)

Template Guidance

If multimedia guidance is populated for the iQ template, it will display on a new page.
The following is a complete list of HTML tags that will render when defined as placeholder text:
  • Hyperlinks: <A href>
  • Fonts: <FONT>
  • Header, style and block elements: <H{n}>, <DIV>, <SPAN>, <P>, <LI>, <HN>
  • Text format: <B>, <I>, <U>, <S>
  • List handling: <OL>, <UL>, <LI>
Any other HTML markup tags will be ignored during report processing. If the HTML represented by placeholder text is not well-formed, the placeholder is rendered as plain text. All HTML tags are case-insensitive.

iQ Questions and Answers

The iQ Questions and Answers displayed are for a Checklist iQ Template Type.
  • Questions and Answers are displayed in the order they are set out in the template.
  • Only Sections and Q&A made visible by branching will be displayed.
  • Questions and Answers will display even if there is no answer recorded in the record.
Title: iQ Template name.
Section Name:
  • iQ Section Name, followed by all the Questions and Answers under that section.
Question Text: The question text.
Question Guidance:
  • Plain text guidance, if present, will be displayed.
  • Multimedia guidance will not be displayed.
Answer: The answer will be displayed in a column to the right of the question.

Images

Show Images can be configured for the report.
Navigate to Settings > System Configuration > Module Setup and select Reports from the module cog menu. Check or uncheck Show Images on Cog Report for the report.
  • If the Q&A has multiple attached images, they will be displayed one after the other in one column.
  • Images will be sorted by create date (oldest to newest).

Comments

Comments will only display alongside a question and answer if there are Comments recorded for that Q&A.

Notes will only display alongside a question and answer if there are Notes recorded for that Q&A.
Notes are ordered by the Note Date (oldest to newest).
