The Audit Detail Report (a set of six Standard System Report variants available within the Procedural Audit module of Evotix Assure) generates a structured, formatted output of a Procedural Audit record, including iQ Template questions and answers, scoring, actions, signatures, images, and notes. The reports are designed to be run against a single Procedural Audit record. Each variant controls which sections are included in the output. Reports are enabled and configured in Module Setup and run from the Cog Wheel Menu on a Procedural Audit record. This article is a cross-module reference: the report draws data from the Procedural Audit record, the associated Project record, the iQ Template (Intelligent Questionnaire Template), and System Settings.
Prerequisites
- The user must have the View Reports Role Permission (a set of access rights defining what actions a user can perform within specified modules and Organisational Units) enabled for the Procedural Audit module to run reports.
- The Reports must be enabled within the Module Setup of the Procedural Audit module before they appear in the Standard Reports list or the record Cog Wheel Menu.

Enabling and Configuring Reports in Module Setup
1. Navigate to Settings > Organisational Configuration > Module Setup.
2. Navigate to the Audit section and click on the Cog menu for Procedural Audit and select the Reports option.
3. From here you can customise the Report title, add or remove Reports from the Standard Reports list and enable images to be shown on the reports from the Cog menu by default.

Cover Page
- The image is sourced from an attachment on the record's associated Project record with description "Report Image". The image will only display if an image is available.
- If there is a report image specified for the record's Org Unit then this will be used. Otherwise the Report image specified in System Settings will be used.
- The iQ Template Name.
- Recommended image ratio 16:9. The image is sourced from an attachment on the record's associated Project record with description "Site Image". If there is no image, the Page Title is vertically centred within the available space.
- The Cover Page fields will only be displayed if they have a value saved in the record. The Cover Page fields will reflect any caption configuration (caption change or display in interface). The Cover Page fields will localise depending on the language setting of the user running the report. The Cover Page fields (apart from Site Address) are sourced from the Audit Record.
- The Site Address is sourced from the "Address" field on the record's associated Project record. If Address is empty, the Org Unit from the Audit record will be shown.
Additional Fields
The Additonal Fields page applies to all report variants
- The page will only be displayed if the iQ Template has additional fields and at least one has a value recorded. Each field will only display if there is a value recorded against it.
- No title is displayed on this page.
- This report is designed to be run against a single record. If run against more than one record, the footer displays the name of the first record's iQ Template, and the page number does not reset for each record.
- Page Footer Title: Displays the name of the iQ Template.
- Page Number: Displays the report page number.
Signatures
- The Signatures page will only show if there are signatures recorded against the record.
- Title: The Signatures title is non-configurable. The Signature title is localised.
- Signature: Signature captions (Full Name, Description, Signature) can be configured via Caption Maintenance. Signature captions are localised.
Question Actions
- The "Question Actions" title is non-configurable. Titles will localise depending on the current user's language setting.
- Actions created against iQ Questions are displayed first. Actions will be sorted and grouped into iQ sections in template order. Within the section, Actions are ordered by Priority (high to low).
- The iQ section name will be a header. The section header (and Actions) will only display if there have been Actions created within that section.
- Action, Action Detail, Priority, Current Status, For User, Category, Due Date. Colour coding of Priority and colour coding of Status are applied as configured in System Settings.
Record Actions
- "Record Actions" title is non-configurable. The "Record Actions" title will localise depending on the language setting of the user running the report. Record-level Actions are sorted by Priority (high to low).

- Output includes a table and a bar chart. Scoring displayed here is for a Mixed iQ Template type. The legacy audit scoring where each question has an "Initial" and "Follow Up" score is also supported.
iQ Questions and Answers

- The iQ Questions and Answers displayed are for a Mixed iQ Template Type. The Questions and Answers are displayed in the order that they are set out in the template. Only Sections and Q&A made visible by branching will be displayed. Questions and Answers will display even if there is no answer recorded in the record.
- The iQ Template name.
- The iQ Section Name, followed by all the Questions and Answers under that section.
- The question text as set in the iQ Template.
- Plain text guidance if present will be displayed. Multimedia guidance will not be displayed.
- If the answer is a short answer (45 characters or fewer), it will be displayed in a column to the right of the question. If the answer is a long answer, it will be displayed as a new row.
Images
Navigate to Settings > System Configuration > Module Setup and select Reports from the module Cog menu to enable images by default.
- Tick Show Images when running the report.
- If the Q&A has multiple attached images, they will be displayed one after the other in one column. They will be sorted by create date (oldest to newest)
Comments
- Comments will only display alongside a question and answer if there are Comments recorded for that Q&A.
Notes

- Notes will only display with a question and answer if there are Notes recorded for that Q&A. Notes are ordered by the Note Date (oldest to newest).
Important Notes
- This report is designed to be run against a single record. If run against more than one record, the footer displays the name of the first record's iQ Template and the page number does not reset for each record.
- The cover page logos and site image are sourced from the associated Project record, not from the Audit Record itself. If no Project record is linked, these images will not appear.
- Cover Page fields localise based on the language setting of the user running the report, not the language of the record.
- Multimedia guidance in iQ Templates is not rendered inline within Q&A sections. Where Template Guidance applies, multimedia guidance is displayed on a separate page.
