This article explains the two most common reasons why a Questionnaire (a simplified data-collection form used in the Portal and AssureGO+) submitted via AssureGO+ (the Evotix mobile web application) does not immediately create a record in the associated Assure module. The two causes are: (1) the Auto-Process Portal Queue setting is not enabled on the Questionnaire, and (2) one or more mandatory module fields are missing from the Questionnaire configuration. Both issues are diagnosable and resolvable from within the Questionnaire configuration area in Assure.
This article spans the Portal and AssureGO+ areas of Assure, with cross-module implications for any module underpinning a Questionnaire (for example, Incident Record, Vehicle Incidents, or DSE Assessment), as fields mapped to a Questionnaire link directly back to the corresponding module record in the main system.
When a Record is submitted via AssureGo+ and does not immediately show in the associated Module, this may be due to the following reasons: 1) the Questionnaire has not been configured to Auto-Process, 2) a record may not be auto-processing is due to missing mandatory fields
Reason 1: The Questionnaire Is Not Configured to Auto-Process
This can be checked and amended in the Questionnaire Configuration by completing the following steps.
Navigate to Settings and locate the Questionnaire option under the Portal section. The highlighted column in the Questionnaire list will confirm whether the Auto Process Portal Queue setting is currently enabled for each Questionnaire.
The Auto Process Portal Queue column in the Questionnaire list view displays either Yes or No for each Questionnaire, indicating whether auto-processing is active.
The Questionnaire has not been configured to Auto-Process. This can be checked and amended in the Questionnaire Configuration by completing the following steps.
How to Enable Auto-Process on a Questionnaire
- Navigate to Settings > Portal > Questionnaire.
- Locate the relevant Questionnaire in the list.
- Click the gear/settings icon (Menu Cog) next to the Questionnaire and select Edit.
- In the edit view, locate the Auto Process Portal Queue checkbox.
- Tick the Auto Process Portal Queue checkbox.
- Click Save & Close.
What does the Auto Process Portal Queue setting do?
If ticked, when submitting this Questionnaire, auto-process will be attempted. If successful, the submission will go directly into the system and skip the Portal Queue. It should be noted that for this feature to work, the Questionnaire must contain all required fields, and any non-direct input field data will be lost. Auto-Process is also applicable to mobile submissions.
Reason 2: Mandatory Fields Are Missing from the Questionnaire
The second reason a record may not be auto-processing is due to missing mandatory fields. This is when a field is mandatory in the Assure form but has not been placed on the Questionnaire. As mandatory data is not populated, the record cannot be created and is placed within the Portal Queue.
How to Check for Missing Mandatory Fields
Follow the same navigation steps as above (Settings > Portal > Questionnaire), open the relevant Questionnaire in Edit view, then locate the Auto Process warning banner. The warning box contains the message: "Auto Process requires mandatory fields to be present in the Questionnaire and all fields marked as direct input. Check missing fields here." The word "here" is a clickable link.
Clicking the link will display which mandatory fields are absent from the Questionnaire. For example, a missing field might be displayed as Date Of Incident.
The missing fields check is accessible in two ways:
- From the Auto Process warning banner within the Questionnaire edit view (click the link labelled "Check missing fields here").
- From the Auto Process Portal Queue configuration section within the same edit view.
How to Resolve Missing Mandatory Fields
Once the missing mandatory field(s) have been identified, the field(s) must be added to the Questionnaire's section configuration. For step-by-step guidance on adding fields to a Questionnaire, refer to the How To Configure Questionnaires article.
Key Concepts
| Term | Definition |
| Questionnaire | A simplified data-collection form used in the Assure Portal and AssureGO+ mobile web application. Shorter than the main module form, it collects key information from users who may not have a full Assure licence. |
| Auto Process Portal Queue | A Questionnaire setting that, when enabled, attempts to create a record directly in the associated module upon submission, bypassing the Portal Queue. |
| Portal Queue | The holding area where submitted Questionnaire responses are placed when auto-processing cannot be completed. Records in the Portal Queue require manual review and processing by an authorised user. |
| Direct Input | A field configuration setting that allows submitted data to be written directly into the corresponding field in the main Assure module record. Fields not set to direct input cannot be auto-processed. |
| Mandatory Field | A field marked as required in the Assure module form. If a mandatory field is not included in the Questionnaire, auto-processing will fail and the record will enter the Portal Queue. |
Notes and Warnings
- Auto-Process requires all mandatory module fields to be present on the Questionnaire.
- Fields that are not set to direct input will cause data to be lost during auto-processing and will prevent the record from being auto-processed successfully.
- Auto-Process applies to both Portal and AssureGO+ (mobile) submissions.
- If Auto Process is enabled and all mandatory module fields are present in the Questionnaire, submitted records are created directly in the module without manual intervention. If mandatory fields are missing or Auto Process is disabled, records enter the Portal Queue for manual review.

- Relevant Modules and Cross-Module Implications: This article applies to the Portal configuration area of Assure. Any Assure module that underpins a Questionnaire (for example, Incident Record, Vehicle Incidents, DSE Assessment, Activity Risk Assessment, or Audit) is affected. Fields mapped to a Questionnaire link directly to the corresponding module record. Auto-Process behaviour is determined per Questionnaire and affects how data flows into the underlying module. Missing mandatory fields from the module form will block auto-processing regardless of which module the Questionnaire is based on.
