In AssureGO+ (the Evotix Progressive Web Application for mobile and desktop form submission), users can annotate any JPG or PNG photo uploaded as an attachment to a Questionnaire (a simplified data-collection form published via a Portal Dashboard). Annotation tools allow users to draw freehand, insert text, arrows, lines, and shapes directly on the image. Annotated images are visible in the submitted record within the Assure main system.Important: Once annotations are saved, they cannot be undone.
Prerequisites
- The Questionnaire must have the Attachments capability enabled. When attachments are enabled on any AssureGO+ form, the image annotation feature is automatically available. No additional configuration is required.
- The image must be in JPG or PNG format. Other formats cannot be annotated.
- The user must be accessing AssureGO+ from a supported browser. Annotation is available on most modern browsers but is not supported on Internet Explorer or iOS Safari version 12 or below.
When to Use Image Annotation
Annotation is useful in the following scenarios:
- Highlighting a hazard: When reporting a hazard or other safety event, the user can draw attention to the specific area of concern directly on the image rather than writing a long description.
- Disguising confidential content: When an uploaded image contains irrelevant or confidential information, the user can cover or obscure that area using a filled shape.
Annotation Tools
When the annotation editor is open, the toolbar displays the following options, listed left to right:

When each tool is selected, a menu to change the colour or line style appears at the bottom of the editor.
Limitations
- Annotation is available for JPG and PNG image formats only.
- Annotation is not supported on Internet Explorer or iOS Safari version 12 or below.
- Once annotations are saved, they cannot be undone. The Undo option is only available before selecting Confirm and save.
- Image annotation can only be performed in AssureGO+. Annotations cannot be applied to images within the Assure main system.
How to Annotate an Image
Step 1: Add an Attachment
- Open the AssureGO+ Questionnaire and navigate to the attachments section.
- Select Add Attachment.
- Choose the image file from the device (camera, photo library, or file browser).
- Select OK to confirm.
Step 2: Open the Annotation Editor
- Once the image is attached, click the three-dot menu on the attachment.
- Select Annotate image.
The annotation editor opens with the image and the toolbar displayed.
Step 3: Annotate the Image
- Select the required tool from the toolbar (see Annotation Tools table above).
- Draw, write, or place the selected annotation element directly on the image.
- Use the colour and line style menu at the bottom to adjust the appearance as needed.
- Use Undo last change to reverse any change before saving.
Step 4: Save the Annotation
- Select the Tick (Confirm and save) button to save the annotation.
Warning: Saved annotations cannot be undone. Confirm the annotation is correct before selecting the Tick.
Step 5: Submit the Form
- Complete the remaining sections of the Questionnaire and click Submit as usual.
- The annotated image will be visible as an attachment in the submitted record within the Assure main system.
Notes
- Annotation is automatically available on any AssureGO+ form that has the Attachments capability enabled. No separate configuration step is needed.
- The Undo function is only available within the annotation session, before the annotation is saved. Once saved, the annotation is permanent.
- Annotated images appear as standard attachments in Assure and can be viewed by any user with access to the submitted record.
