Open the Assure Go+ App on your phone. Navigate to the cog wheel and select the language setting.
Select the 3 dots on the form you wish to complete.
Select Start Form
When the form opens – on the left-hand side of the screen you will see the Sections that are available on this form. You can also use this to Navigate to different sections of the form and the IQ Template.
First select the Org Unit. Then select ok.
On a date field – if you select the Today button it will populate the date you are filling in the form. However you can manually add in the date.
On the time field – you can select the Now button and it will populate the current time you are filling out the form. As with the date field you can manually add in the time and select whether it is AM or PM.
Picklist will show an arrow that can expand the dropdown boxes and will display differently depending on how many options there are within the picklist.

When you complete a Section, you can select the next button.
You are also able to add Actions on the Assure Go+ forms.
You can use the Add Attachments option to add attachments onto the Form on Assure Go+.
Please Note - We can no longer support JIFI file types.
Sometimes a Validation error will appear when you try to submit the form – this can mean there maybe missing Mandatory Data, or Inaccurate Data. Selecting ok will navigate you to those errors.
Then select submit and the form will either go to the Portal Queue or straight onto the Main System depending on how you have configured the Questionnaire.
