This article explains how to select, open, and complete a Questionnaire (a simplified data-collection form configured in Assure and published via a Portal Dashboard) within AssureGO+ (the Evotix Progressive Web App for mobile and desktop form submission). It covers opening the app, finding and sorting forms, selecting an Org Unit, completing each field type, adding Actions and Attachments, handling validation errors, and submitting the record. On submission, the record either auto-processes directly into the Assure module or enters the Portal Queue for manual review, depending on how the Questionnaire has been configured.
Cross-module scope: Questionnaires submitted through AssureGO+ write records into the corresponding Assure module (for example, Incident Record, Audit, Activity Risk Assessment). Actions created during form completion are managed within the Actions area of Assure.
Prerequisites
- Access to a published AssureGO+ portal URL or installed app shortcut.
- A supported browser: Google Chrome, Microsoft Edge, or Safari (latest or one version prior).
- An internet connection (required to load the form initially and to submit). Offline completion is available if the Questionnaire has been saved for offline use.
Completing a Form Step by Step
Step 1 - Form Options
Open the AssureGO+ Portal app on the device. To change the langauge, select the cog wheel icon in the top right hand corner. The Language selector will only appear if the system has additional languages configured.
To find the form to complete:
- Search for the form using the search bar at the top of the home page.
- Alternatively, use the Sort dropdown to display forms in a different order. The default option shows forms in the order they were added to the Portal Dashboard. Two alphabetical sort options are also available. Click the downward arrow menu on the form to display options, including the option to Download for offline use. This stores a copy of the form locally on the device. Once the device reconnects to the internet, the completed form will upload to the main system.
Step 2 - Start the Form
To start the form, simply click on it and a pop-up window will appear asking you to confirm. Click Start Form.
When the form opens, the left-hand navigation panel displays all available sections. This panel can be used to navigate directly to any section of the form, including any attached iQ Template sections.
Step 3 - Select the Org Unit
The first step in the form is to select the Org Unit (the node in the Assure organisational hierarchy that determines the reporting structure and record destination). Click the Org Unit field to open a search modal. Search by name or browse through the hierarchy to locate the correct Org Unit. Select the chosen Org Unit and click OK.
Step 4 - Complete the Form Fields
Field types vary depending on how the Questionnaire has been configured. The following field types may appear:
Date Fields
Click the Today button to automatically populate the current date, or enter the date manually. Date fields will return a validation error if inaccurate information is entered, for example if a future date is entered in a field where one is not permitted.
Time Fields
Click the Today button equivalent for the time field (the Now button) to automatically input the current time. UK format displays as HH:MM (24-hour). US format displays with an AM/PM selector.
Picklists
Picklists display a dropdown where one of multiple responses can be selected. Picklists with more than five items display as a dropdown. Picklists with five items or fewer display as radio buttons.
Text Fields
Any text can be entered. The character limit is shown in the field caption.
Numeric Fields
Only numeric values (whole or decimal) are accepted. A validation message appears if non-numeric characters are entered.
Boolean Fields
Yes/No or True/False questions. Only one selection is permitted.
Registers (Person and Equipment)
Registers can be displayed in AssureGO+ forms. To select a person from the Person Register, click Select Person Reference to display the list of available people. To select equipment from the Equipment Register, click Select Equipment Reference to display the asset list. Alternatively, select Enter Manually to type the reference directly.
Note: Register selection requires the user to be online and logged in. Offline or unauthenticated users will only see the Enter Manually option.
Signatures
If the form includes a signature field, click Add Signature to open the signature input area. Once the signature has been entered, click Next to move to the next section.
Step 5 - Assign the Record (If Enabled)
AssureGO+ allows a record to be assigned to a user before submission. To assign a record, select Assigned to display the list of available users. Select the user and click OK.
Note: This feature must be enabled in the Questionnaire configuration and requires the user to be online and logged in.
Step 6 - Add Actions (If Enabled)
If Actions are enabled on the Questionnaire, select Add Action to create a follow-up task. Complete the action details. When assigning the action, two options are available: Use Default User (selects the default action recipient for the selected Org Unit) or Select User (assigns the action to a specific user who has an Assure or AssureGO+ licence). Select the user and click OK. Click OK to save the action.
Step 7 - Add Attachments (If Enabled)
If attachments are enabled, click Add Attachments. Optionally enter a description for the attachment, then click OK.
On desktop, the device File Explorer opens. On mobile, the user is offered the option to take a photo or video, access the photo library, or browse files.
Note: JIFI file types are not supported.
To annotate an image attachment, click the three-dot menu on the attachment and select Annotate image. Annotation options include shading out areas, adding text boxes, and drawing arrows. Click the tick button when annotation is complete.
Step 8 - Navigate Between Sections
Click Next to move to the following section of the form. The left-hand navigation panel can also be used to jump directly to a specific section at any time.
Step 9 - Submit the Form
Click Submit once all sections of the form are complete. Submit will only be avaliable from the last section of the Questionnaire.
Validation Errors
Validation errors will occur on submission if mandatory fields have not been completed. Click OK to acknowledge the error. The mandatory field that was missed will be highlighted at the top of the section, in the left-hand navigation panel, and on the specific question that was missed. Correct the missing data and click Submit again to complete the form.
After Submission
The form will either go directly to the Assure module or enter the Portal Queue, depending on how the Questionnaire has been configured.
- If Auto Process is enabled and all mandatory fields are present, the record is created directly in the Assure module.
- If Auto Process is not enabled, or mandatory fields are missing, the record enters the Portal Queue (the holding area where submitted Questionnaire responses are placed for manual review and processing by an authorised user).
Notes
- Offline use: If the Questionnaire has been saved for offline use, it can be completed without an internet connection. Submission will be queued and will sync automatically once connectivity is restored.
- Draft / In Progress records: If the user navigates away from the form after entering data, the data is automatically saved in a draft state and can be found in the In Progress tab on the Home Page.
