Within Assure, the Portal Dashboard (the central interface for managing AssureGO+ portal apps) includes a built-in QR code that directs users to the AssureGO+ Portal Dashboard homepage. This article covers a different use case: generating a QR code that links directly to a specific Questionnaire (a simplified data-collection form published via the Portal Dashboard) so that when the code is scanned, the Questionnaire opens immediately without the user needing to navigate from the homepage.
This is achieved by copying the direct URL of the Questionnaire from AssureGO+ and entering it into an external QR code generator tool.
Prerequisites
- Access to the AssureGO+ Portal URL.
- The Questionnaire must be published and visible on the Portal Dashboard.
- A browser that supports right-clicking on links (for example, Google Chrome, Microsoft Edge, or Safari on desktop).
Steps
Step 1: Open AssureGO+ and Locate the Questionnaire
Open AssureGO+ in a browser and navigate to the Portal Dashboard homepage. Identify the Questionnaire for which a direct QR code is needed.
Note: Do not click the Questionnaire to open it. Right-click the Questionnaire link/button directly on the homepage.
Step 2: Copy the Direct Link to the Questionnaire
Right-click on the Questionnaire. From the context menu that appears, select Copy Link Address (this option may appear as "Copy link" depending on the browser).
This copies the direct URL for the Questionnaire to the clipboard.
Step 3: Generate the QR Code Using an External Tool
Open an external QR code generator. Paste the copied Questionnaire URL into the generator to produce a QR code.
Tip: Evotix does not provide a built-in QR code generator for individual Questionnaires. Any reputable external QR code generator can be used, such as https://www.qr-code-generator.com/free-generator/.
Step 4: Share the QR Code with Your Organisation
Download or save the generated QR code. The QR code can then be shared across your organisation by printing it and displaying it on-site, or distributing it digitally.
When a user scans the QR code, AssureGO+ will open directly on the relevant Questionnaire, ready for the user to complete and submit.

- Questionnaire URL changes: If the Portal Key for the Portal Dashboard is ever changed in Assure, the Questionnaire URL will also change. Any QR codes previously generated from the old URL will stop working and will need to be regenerated using the new URL.
- Offline access: Scanning a QR code requires an internet connection to load AssureGO+. The Questionnaire can subsequently be saved for offline use once opened.
