This article explains the conditions under which users can add Actions, Attachments, and Notes to questions within an iQ Template (Intelligent Questionnaire Template -- a configurable question set used to capture structured data within Assure modules such as Audits, Risk Assessments, and Inspections). Three enablement flags must be switched on at the question level during iQ Template configuration before users can interact with these features. Additionally, the Questionnaire must be in an In Progress status for these options to be accessible. This article is relevant to iQ Template administrators and end users completing Questionnaires in Evotix Assure.
Prerequisites
Before Actions, Attachments, or Notes can be added to an iQ Template question, the following conditions must both be met:
- The relevant settings must be enabled on the individual iQ Template question (see Configuration section below).
- The iQ Template Questionnaire must have an In Progress status.
Important: The iQ Template must be in Draft mode before any question-level settings can be updated.
Configuration: Enabling Actions, Attachments, and Notes at Question Level
An administrator must enable the following settings individually on each iQ Template question where the capability is required. These settings are configured within the iQ Template Configuration screen.

Note: To update these settings, the iQ Template must first be placed into Draft mode. Navigate to Settings > Templates > iQ Templates, select the Menu Cog (cog wheel icon) on the relevant template, and choose Create Draft before making any configuration changes.
Questionnaire Status Requirements
Actions, Attachments, and Notes can only be added to iQ Template Questionnaires that are In Progress.

User Experience: Interacting with Questions
When a user expands a question within an In Progress Questionnaire, icons for the enabled capabilities (Actions, Attachments, Notes) are displayed alongside the question. The Add Web Link option is always visible on every question, regardless of the Allow Actions, Allow Attachments, or Allow Notes settings.
Actions
- Actions added to an iQ Question are displayed in a list view format.
- The list displays up to 50 Actions before paging occurs.
- Users can Edit or Remove Actions using the icons visible in the list view.
Attachments
- Attachments added to an iQ Question are displayed in a table.
- Each attachment row displays Download, Edit, and Delete icons.
Notes
- Notes added to an iQ Question are displayed in a list view format.
- The list displays up to 50 Notes before paging occurs.
- Users can Edit or Remove Notes using the icons visible in the list view.
Note: The Actions, Attachments, and Notes sections are only visible on a question if at least one item has already been added to that question. They do not appear as empty sections by default.
